The Community Care Assistant works with Vancouver Coastal Health (VCH) at the Home Support team in Vancouver, BC, providing home support services to clients within a client and family centered care model. This role involves assisting clients with varied levels of care, observing and reporting significant changes in their presentation, and collaborating with the health care team, clients, and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs). The assistant motivates, encourages, and involves clients in their care and provides input and feedback in the care planning process to the health care team.
