About The Position

Integrated Services for Behavioral Health (ISBH) is seeking a Housing Community Behavioral Health Worker in Franklin County, OH. ISBH is a community-minded, forward-thinking behavioral health organization dedicated to helping individuals on their path to health and well-being. We provide services in homes and communities, connecting individuals with necessary resources. Serving Southeastern and Central Ohio, we offer a comprehensive range of behavioral health and other services, collaborating with local partners to foster healthy people and strong communities. All services are designed to be collaborative and personalized. This role is part of a team focused on assisting individuals reentering the community from incarceration in central Ohio. The worker will collaborate closely with clients and housing specialists to achieve permanent housing stability using federal, state, and local resources. The pay range for this position is $19.00-$22.26 hourly, based on experience.

Requirements

  • Minimum three years of directly relevant experience or a Bachelor’s degree preferred.
  • High School Diploma.
  • Demonstrated a high degree of cultural awareness.
  • Experience with multi-need individuals and families.
  • Broad knowledge of community service systems.
  • Licensed and able to operate a motor vehicle with an appropriate level of insurance coverage.
  • Willing to participate in and lead cross-systems team-building activities.
  • Able to effectively communicate through verbal/written expression.
  • Must be able to operate in an Internet-based, automated office environment.

Nice To Haves

  • Experience with individuals experiencing homelessness preferred.

Responsibilities

  • Assesses or defines the strengths and needs of referred people, families, and communities.
  • Directly provides home and community behavioral health services that are practical, helpful, and collaborative.
  • Meets people wherever they may be with support for self-management of health and well-being.
  • Carries out utilization review and quality assurance activities as directed.
  • Maintains necessary documentation, participates in program evaluation, attends team and program planning meetings, cross-systems training, and acquires knowledge of community resources.
  • Meets billing productivity requirements established by Integrated Services for Behavioral Health.
  • Other duties as assigned.
