About The Position

The Community Based Vocational Counselor provides counseling interventions to People We Support in the Career Solutions program. The role develops vocational plans for OPWDD services, including SEMP, Pathway, and ETP, and completes NYESS data entry for programs as needed. The counselor monitors referrals, conducts intakes with Employment Specialists, maintains and reviews documentation, supports service delivery, provides benefits advisement, and participates in investigations of Reportable Incidents and Allegations of Abuse. The position has no supervisory responsibilities and requires flexibility to meet program needs.

Requirements

Bachelor’s degree required.
Valid and acceptable New York State driver’s license required.
Authorized to work in the United States.

Nice To Haves

Degree in Human Services or a related field preferred.
One to two years of experience working with people with developmental disabilities preferred.

Responsibilities

Provide counseling interventions as needed to all People We Support in the Career Solutions program.
Develop vocational plans for OPWDD services, including SEMP, Pathway, and ETP.
Complete NYESS data entry for all programs as needed.
Monitor and track referrals within OPWDD guidelines.
Conduct intakes with Employment Specialists for individuals.
Complete case file reviews, satisfaction surveys, referrals, and other internal documentation.
Review daily and monthly notes for waiver and non-waiver services.
Participate in team meetings as scheduled.
Review and approve monthly billable services.
Complete admission and discharge documentation for all OPWDD individuals.
Provide classroom activities to individuals in OPWDD contracts.
Provide guidance to Employment Aides on service delivery and documentation.
Provide benefits advisement to referred individuals.
Participate in the ongoing rotation for investigations of Reportable Incidents and Allegations of Abuse.
