Community Based Therapist
About The Position
Nexus-FACTS is currently hiring for a HYBRID REMOTE mental health therapist for their Community Based Programs, SFT (Systemic Family Therapy) and CIBS (Collaborative and Intensive Bridging Services). SFT is an intensive in-home model of service intervention specific to Nexus-FACTS designed to keep at-risk youth in their homes and community. This model provides multiple therapeutic contacts per week with the family system, including individual, family, parent, and couples’ therapy, exceeding what youth and family would typically receive through MA or insurance billable therapy services. The Community-Based Therapist provides mental health therapy to children, adolescents, adults, and families in homes, community settings, via telehealth, and at Nexus Family Healing offices. The role utilizes a trauma-informed, strength-based approach, delivering individualized care through evidence-based models such as EMDR, DBT, TFCBT, and Systemic Family Therapy.
Requirements
- Master’s degree in one of the behavioral sciences, human services, social work, marriage & family therapy, psychology, counseling, or clinically related fields.
- If social worker, must have LGSW in Minnesota.
- If currently unlicensed: Must be on track to complete licensure for a specific, related discipline, And/or currently holds a provisional/intermediary license for a specific track (i.e. LGSW, LAMFT or LPC).
- Valid driver’s license required.
- Must meet state regulating agency and Nexus Home Office driving requirements.
- Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
Nice To Haves
- Fully licensed in Minnesota as an LICSW, LMFT, or LPCC.
- Related residential or human services experience working with youth and families.
Responsibilities
- Complete intake forms and diagnostic assessments.
- Ensure timely and accurate clinical documentation.
- Complete CANS assessments using TCOM principles.
- Develop, update, and monitor individual treatment plans (ITPs) based on assessments and CAT/CANS.
- Provide crisis intervention and safety planning as needed.
- Participate in mandatory clinical supervision and team case consultation meetings.
- Participate in family collaborative intake meetings.
- Provide client-centered, strength-based therapy in collaboration with families.
- Deliver individual, group, and family therapy aligned with treatment goals.
- Meet direct service and documentation requirements.
- Support discharge planning and post-care coordination.
- Coordinate referrals and collaborate with families, schools, and providers.
- Attend supervision, team meetings, and required trainings.
- Maintain compliance with all clinical and regulatory standards.
- Communicate client needs with care teams, families, and community partners.
- Ensure confidentiality and HIPAA compliance.
