About The Position

Home Health Care, Inc. is seeking a Community-Based Physical Therapist (PT) to join their team. This role offers the opportunity to treat one patient at a time with full attention, create personalized care plans based on the patient's living environment, build relationships to see meaningful progress, and enjoy the flexibility and independence of managing your own day. Therapists will practice at the top of their license with strong interdisciplinary support. Requirements

• Active Minnesota Physical Therapist (PT) license.
• Strong clinical reasoning and patient-centered approach.
• Excellent communication and relationship-building skills.
• Ability to work independently while managing a flexible field schedule.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients in their homes and establish individualized treatment plans.
• Improve mobility, balance, strength, endurance, and functional independence.
• Recommend adaptive equipment and home safety modifications when appropriate.
• Educate patients and caregivers to maximize long-term outcomes.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team.
• Maintain timely and accurate clinical documentation.
