About The Position

The Community Based Clinical Support Specialist II is part of a cross-trained team providing support to Choptank Community Health (CCHS) Community Based program, including School Based Medical and Dental (SBHC), Migrant and Mobile Health Unit (MHU) programs. The Community Based Clinical Support Specialist II provides a range of clinical, administrative, and staff support to a school based medical operation. Duties may include the following: taking medical histories, taking vital signs, assisting during patient/procedures/examinations, collecting and preparing laboratory specimens, using various pain scales to screen for pain, EKGs, and nebulizer treatments, disinfection and stocking of exam rooms and clinical areas, as well as the administrative SBHC duties. Assists in the performance of medical and administrative procedures; prepares and maintains medical equipment and supplies in accordance with established protocol, procedures, policies, and standards, and assists in the coordination of day-to-day patient administration, records management, and medical support activities. This position requires traveling to various sites to function as support for the Community Based Medical & Dental providers including MHUs.

Requirements

  • CPR certified. If not certified, can complete the certification during orientation period.
  • A valid driver’s license, as travel to multiple sites is a requirement.
  • Drivers’ training for CCHS vehicles including MHU will be provided as driving CCHS vehicles (MHU) is a requirement of this position.
  • Must be proficient in computer and basic math skills.
  • Demonstrated experience providing exceptional customer service consistent with AIDET.
  • High School Diploma or equivalent
  • Certificate of completion from an accredited medical assistant program. Certification must remain current during the entire course of employment.
  • BLS certification required.

Nice To Haves

  • Bilingual language skills (Spanish/English) a plus
  • Minimum six months’ experience preferred.
  • Preferred experience in various medical settings to include family practice, pediatrics, and women’s health.

Responsibilities

  • Greets the patient and provides quality customer service in person and on the telephone. Monitors voicemail on an ongoing basis.
  • Prepares, distributes, and collects SBHC enrollment forms each school year, ensuring that enrollment forms are completed adequately by parents/guardian and takes steps to collect needed information including insurance coverage.
  • Updates the Practice Management System to accurately reflect patient demographics and insurance information.
  • Posts charges and payments to the practice management system.
  • Works closely with the Billing Department to correct billing errors.
  • Verifies insurance information and assists with enrollment in sliding fee.
  • Completes pre-registration, registration, appointment confirmation, and scheduling.
  • Maintains and submits monthly forms and inventory and accurately documents and update logs according to SBHC policy and procedures.
  • Controls the patient flow for services, including set up and maintenance of all SB Program care spaces.
  • Restocks community-based sites with supplies and paperwork without prompting.
  • Ensures the patient visit encounters and notes home are completed and routed as appropriate.
  • Prepares patients properly for the clinician by following established intake standards for all visit types.
  • Assists clinician with coordination of care between the medical, dental, nutrition and behavioral health staff.
  • Cleans and disinfects the SBHC exam space between patients, adhering to the Cleaning and Disinfection schedule.
  • Dons and doffs PPE properly as required for each type of patient encounter.
  • Assists in the maintenance of all stocked medicines, medical equipment, including vaccines and CLIA waived tests in the SBHCs.
  • Completes training and/or competencies to include completion of vital signs, BLS CPR/AED, autoclave/sterilization, (infection control here– i.e.: handwashing and donning and doffing PPE) and medicine management to support clinical care in the SBHC.
  • Participates in school and community activities to promote the SBHC program.
  • Communicates with school personnel, Choptank Community Health System Administration, and co-workers for schedule changes, closings, etc.
  • Covers shortages within department including rotating to various school sites to complete scheduled services.
  • Regular, reliable attendance is a requirement of this job.
