About The Position

The Community-Based Care Coordinator assists the Behavioral Health Manager in effectively advocating and coordinating services and resources for individuals experiencing behavioral health related challenges. This is achieved by providing support and coordination to individuals after calls for service, discharge from incarceration, transitions from Bitterroot Health and/or West House and re-entry into the community. The Care Coordinator will work closely with community partners and the Ravalli County Behavioral Coalition. This position is grant funded and dependent on continued future funding.

Requirements

Experience and skill in establishing and maintaining effective working relationships with various governmental entities, law enforcement agencies, criminal justice system staff, citizens, elected officials, and businesses.
Professional appearance and hygiene in accordance with RCSO policy.
Excellent written and oral communication skills.
Responsive to client and co-worker needs, including being tactful and considerate of others regardless of their status.
Manage difficult clients in a professional manner.
Exhibit sound and accurate judgement.
Demonstrate flexibility and the ability to quickly adapt to change in the work environment.
Observe safety and security procedures and report any potentially unsafe conditions.

Nice To Haves

High school diploma, and any combination of education and experience that qualifies the employee to deal with social work, human services, crisis de-escalation, and behavioral health clients.

Responsibilities

Advocate for clients
Organize referrals.
Identify and coordinate client goals, strengths, and needs by providing outreach and support in attaining client goals.
Assist in establishing care with medical and psychiatric providers.
Assist in locating information and resources for short term monetary needs.
Prepare accurate and concise data reports.
