About The Position

Compassionate, collaborative, and engaging Coaches are needed at The Evolve Youth Time Limited Rental Assistance Program. The position is designed to work with individuals experiencing homelessness to find rapid housing with private landlords. Coaches provide guidance, coaching, and support to vulnerable young adults who are at risk of homelessness, to help them achieve permanent housing with the support of time limited rental assistance. Coaches will adopt a nonjudgmental, strengths-based, and collaborative approach to working with youths, ensuring that their wishes, interests, and preferences inform the development of their service support plan goals. Coaches will collaborate with their team to ensure that youth are well equipped to navigate independent living. This will include crisis intervention entitlements procurement, health care, mental health, independent living skills, job/career assistance, financial literacy, recreational opportunities, education journey and other services as needed. Coaches will use the principles of positive youth development to ensure that youths’ goals and services are focused on building independent living skills, resiliency and social connection which will ultimately ensure independence. Coaches will empower youth to engage in all aspects of the process and teach them how to advocate for themselves and prevent or troubleshoot obstacles. Coaches will develop a robust network of service providers to ensure that youths are able to access the services that they desire. Coaches will be trained in and implement evidence-based models, including: Motivational Interviewing Intensive Trauma Services Recovery Oriented and Trauma Informed case management · Critical Time Intervention Coaches will undergo a comprehensive training program to ensure that they are trained in evidence-based practices, as well as in conflict resolution, crisis intervention, crisis de-escalation, harm reduction, person-centered planning, assessment, housing first approaches, mandatory reporting and recognizing signs of abuse and neglect, co-occurring disorders, and recovery-oriented practices.

Requirements

  • Experience assisting with obtaining and maintaining government benefits, accessing services, managing medication, and comfort running group meetings when necessary.
  • Ability to act responsibly in stressful/crisis situations.
  • Written and verbal communication skills sufficient to effectively provide direction and counseling to clients and maintain required documentation.
  • Must be capable of and willing to make home visits unassisted throughout the greater New York area, and to ensure a realistic and secure approach to such visits.
  • Must be capable of independently and professionally assessing the physical and emotional well-being of the client and ascertaining the adequacy of the physical environment and treatment/educational/vocational programs.
  • Ability to independently organize time and prioritize responsibilities in order to plan and carry out a demanding schedule.
  • Valid New York State driver’s license, to provide transportation when required and to handle certain crisis.

Nice To Haves

  • Knowledge of group dynamics and interaction, especially with the adolescent populations, is preferred.
  • Previous experience in social services field or supportive housing is preferred.

Responsibilities

  • Completes Intake process with youth according to established guidelines and makes appropriate contact with and /or referrals to other agencies.
  • Completes needs assessment including but not limited to mental health and social history.
  • Obtains legal documents (birth certificates, NYS Identification, social security), assists with housing applications, assists with employment, budgeting, and enrolls in school where applicable.
  • Facilitates, develops, and participates in skill building groups etc…
  • Provides bi-weekly individual coaching to each youth.
  • Makes referrals to outside services when needed such as substance abuse, mental health, etc..
  • Keeps updated chart records for all current and past residents and completes all mandatory paperwork required by contracts and funding sources.
  • Works alongside all other staff to develop and evaluate programming.
  • Conducts outreach presentations as needed and assigned.
  • Conducts monthly home visits in accordance with CTI Model.
  • Attends regular staff meetings and trainings to support the needs of the youth.
  • Uploads support plans, and relevant documentation that reflects CTI goals and stages.
  • Maintains an active caseload of 15 + tenants.
  • Manages tenant referrals and follows up with phone calls.
  • Cultivates and fosters working partnerships with community landlords.
  • Prepares and tracks documentation including tenant services via Eccovia Data System.
  • Participates in in-service trainings and on-site workshops as required for professional growth and development.
  • Attends external trainings and workshops at direction of management.
