About The Position

This position provides essential support to residents in performing daily living activities, including lifting, transfers, assisting with personal care, and monitoring vital signs. The role involves observing and documenting resident conditions, responding to resident needs, and maintaining a safe and clean environment. Communication with nurses regarding resident status and concerns is crucial, as is timely documentation. Companionship and support are also key aspects of this role, along with meeting federal and state in-service requirements.

Requirements

  • Must meet all applicable state and federal requirements for this position
  • Be able to make independent decisions and follow instructions
  • Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public
  • Communicate effectively in a manner that is sufficient for effective communication with supervisors, team members, prospects, residents, and families
  • Ability to lift fifty (50) pounds.
  • Moving, lifting or transferring of residents may involve lifting up to 75 pounds
  • Ability to exert maximum muscle force to lift, push, pull or carry patients
  • Ability to use abdominal and lower back muscles over time without fatigue
  • Ability to stand for extended periods of time
  • Ability to bend, stoop, stretch, twist, sit and reach
  • Fine motor skills
  • Executes tasks independently
  • Ability to express yourself clearly and effectively verbally
  • Ability to concentrate on moderate details with frequent interruptions
  • Visual acuity
  • Hearing alertness

  • Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility

Responsibilities

  • Provide physical support to assist residents to perform daily living activities, such as lifting, transfers, assisting with bed mobility, bathing, dressing, toileting, standing, walking, exercising, serving and assisting with meals, and taking vital signs
  • Observes and documents the health, behavior, physical and emotional condition of each resident, promptly reporting all changes to the nurse
  • Responds promptly and courteously to resident call signals, signal lights, bells, or intercom systems to determine residents' needs
  • Promotes personal safety and the safety of the residents with a clean and safe environment for residents by observing infection control practices, following facility guidelines, and reporting unsafe situations to the nurse
  • Communicate with nurses on resident conditions, changes and concerns
  • Complete required documentation in a timely manner
  • Provide companionship, comfort and support to residents
  • Meet federal and state in-service requirements
  • Perform other job-related duties as assigned
