About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves ensuring residents receive necessary care, maintaining a safe and comfortable environment, and accurately documenting all services provided. The position requires adherence to facility policies, procedures, and ethical standards, including maintaining resident confidentiality and reporting any changes in resident condition or incidents promptly.

Requirements

  • Must possess, as a minimum, an 10th grade education.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
  • CPR certification.

Responsibilities

  • Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
  • Identify residents using wristbands or photo cards before administering treatments or serving meals.
  • Use only authorized abbreviations when recording information.
  • Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
  • Record all entries on flow sheets, notes, and charts in an informative and descriptive manner.
  • Report all observed accidents and incidents as soon as they occur.
  • Maintain resident confidentiality and report suspected or known violations.
  • Report any known or suspected unauthorized attempts to access the facility’s information system.
  • Perform only trained nursing care procedures.
  • Ensure resident rooms are ready for new admissions (bed made, name tags, admission kit available).
  • Greet residents and escort them to their rooms.
  • Introduce residents to roommates, other residents, and personnel.
  • Make residents comfortable.
  • Inventory and mark resident’s personal possessions.
  • Store resident’s clothing.
  • Assist residents with packing personal possessions during transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, personal, and positive atmosphere, ensuring a calm environment.
  • Meet with nursing personnel to identify and correct problem areas and improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds and put extra covers on beds as requested.
  • Measure and record resident’s temperature, pulse, and respirations (TPRs).
  • Weigh and measure residents as instructed.
  • Ensure residents who are unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements and informing them about food on their tray.
  • Record resident’s food/fluid intake and report changes in eating habits.
  • Keep resident water pitchers clean, filled, and within reach.
  • Perform after-meal care (remove trays, clean hands, face, clothing).
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental/mouth care, bath, hair combing, dressing/undressing).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Check residents routinely to ensure personal care needs are met according to their wishes.
  • Observe and report pressure areas and skin breakdowns to prevent bedsores.
  • Report injuries of an unknown source, including skin tears.
  • Observe disoriented and comatose residents and record/report data.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Maintain confidentiality of all resident care information, including protected health information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident's refusal of treatment requests and report such requests to the supervisor.
