About The Position

The overall purpose of the Certified Nursing Assistant (CNA)/Transportation position is to provide each of the assigned residents with routine daily nursing care and services in accordance with the residents’ plan of care and coordinate patient transportation. This role involves performing various tasks within a nursing home facility, including providing personal care, ensuring resident safety, maintaining a clean and orderly environment, and completing necessary documentation. Additionally, the position requires transporting elderly and rehabilitation patients to medical appointments locally.

Requirements

  • High School diploma or equivalent required.
  • 3-5 years of experience in a skilled nursing or post-acute care setting.
  • Minimum three years professional work experience.
  • Ability to work with colleagues, physicians, and other health care professionals.
  • Basic computer skills required.
  • Strong communication and organizational skills.
  • Exceptional commitment to customer service and employee well-being.
  • Current and valid Driver’s License.

Responsibilities

  • Provides personal care to residents in a manner conducive to their safety and comfort consistent with facility policies and procedures as well as state/federal guidelines and regulations.
  • Strives to provide for resident preference and choice in all areas of care.
  • Identifies and corrects safety hazards in patient care areas and understands personal responsibility for all aspects of patient safety.
  • Complies with established policy, practices, and standards for nursing services.
  • Promotes and supports the greatest possible degree of independence for residents.
  • Advises charge nurse of residents’ status at beginning and end of shift.
  • Makes frequent rounds on unit assigned to assure the environment is neat, odor-free, and orderly and corrects as needed. This includes resident rooms, hallways, public restrooms, shower rooms, dining rooms and any other common areas.
  • Completes daily documentation as required including restorative programming, and computerized documentation.
  • Ensuring compliance with state and federal regulations.
  • Working with the elderly and rehabilitation patients, primarily pickup and delivery to physicians’ offices and hospitals and dialysis, locally.
