About The Position

As a Transport Driver/CNA, you’ll help residents safely access care while providing hands-on support and compassion every step of the way. This role involves split duties 50/50 between floor CNA responsibilities and transport driving. You will schedule and/or provide transportation for residents to and from appointments, assist residents safely during transport and appointments, and drive a center van (similar in size to a minivan) transporting a maximum of two patients at a time. You will also provide resident care according to an approved Care Plan, assist with daily activities, personal care, and resident safety, maintain and respond to resident medical record requests, follow infection control procedures and safety protocols, complete required documentation, including vitals, and report any violations of the Residents’ Bill of Rights immediately.

Requirements

• Completed CNA course and active state license
• Must be 25 years of age or older
• Clean driving record
• High school diploma or GED
• Strong written and verbal communication skills
• Service-oriented mindset with a willingness to learn
• Positive, can-do attitude and excellent attendance

Nice To Haves

• Medical Records experience in a long-term care setting
• ART certification
