About The Position

Empath Health is seeking a compassionate part-time CNA/Program Aide to join its interdisciplinary PACE team in Pinellas Park, Florida. Under the direction and supervision of the Day Center Supervisor, the CNA/Program Aide provides and assists participants with activities of daily living, restorative care, and supportive care. PACE (Program of All-Inclusive Care for the Elderly) helps older adults remain independent and safe in their own homes. The position supports relationship-based care and participant independence and mobility, and collaborates with an interdisciplinary team focused on prevention and holistic well-being. The schedule is Monday through Friday, 11 a.m. to 3 p.m.

Requirements

High school diploma or equivalent.
Current Florida Certified Nursing Assistant (C.N.A.) certification.
Valid CPR certification from the American Red Cross or American Heart Association.
Completes continuing education as required for licensure.

Responsibilities

Consults daily with the Home Care Coordinator or designee about participant status and changes to implement in the plan of care.
Provides personal hygiene care, including skin, mouth, and hair care, and bathing.
Assists participants in and out of bed and with ambulation.
Assists with prescribed exercises taught to the participant and aide by appropriate personnel.
Prepares meals, including therapeutic diets, and assists participants with meals.
Assists participants with using the bathroom, commodes, bedpans, or urinals.
Reports changes in participants’ conditions and needs to the Registered Nurse.
Completes appropriate documentation of services provided.
Maintains confidentiality of company procedures, results, and information about participants, clients, or families.
Participates in continuing education classes and required staff and training meetings.
