About The Position

Iva Post Acute is a beautiful 60-bed skilled nursing and rehabilitation facility located in Iva, South Carolina. The facility is seeking dedicated, dependable, and upbeat Certified Nursing Assistants (CNAs) to join their team. The role involves providing routine daily nursing care and services to residents in accordance with their assessment and care plan, under the direction of supervisors. The facility is undergoing positive changes under new management and offers a warm, supportive work environment with opportunities for growth.

  • Must possess, as a minimum, an 10th grade education
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.

Responsibilities

  • Provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors.
  • Assist residents with activities of daily living (e.g., bathing, dressing, grooming).
  • Provide compassionate, high-quality care to residents.
  • Help create a safe, comfortable, and positive environment for residents.
  • Work closely with nurses and the healthcare team.
  • Build meaningful relationships with residents and their families.
  • Use the wristband or photo card file to identify residents before administering treatments or serving meals.
  • Use only authorized abbreviations established by the facility when recording information.
  • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse promptly.
  • Record all entries on flow sheets, notes, and charts in an informative and descriptive manner.
  • Report all accidents and incidents observed during the shift.
  • Maintain the confidentiality of resident's protected health information and report any suspected or known violations.
  • Report any known or suspected unauthorized attempt to access the facility’s information system.
  • Perform only trained nursing care procedures.
  • Ensure resident rooms are ready for new admissions (bed made, admission kit available, etc.).
  • Greet residents and escort them to their rooms.
  • Introduce new residents to roommates, other residents, and personnel.
  • Make residents comfortable.
  • Inventory and mark resident’s personal possessions and store clothing.
  • Assist residents with packing personal possessions for transfers or discharge.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, and a calm environment.
  • Meet with nursing personnel to identify and correct problem areas and improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds and add extra covers as requested.
  • Measure and record residents' temperatures, pulses, and respirations (TPRs).
  • Weigh and measure residents as instructed.
  • Ensure residents who are unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements (e.g., explaining food placement for visually impaired residents).
  • Record resident's food/fluid intake and report changes in eating habits.
  • Keep residents' water pitchers clean, filled with fresh water, and within reach.
  • Perform after-meal care (e.g., remove trays, clean hands/face/clothing).
  • Perform all assigned tasks in accordance with established policies, procedures, and supervisor instructions.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental/mouth care, bath, hair combing, dressing/undressing).
  • Keep residents dry by changing clothing and linens when wet or soiled.
  • Assist residents in preparing for medical tests (lab work, x-ray, therapy, dental).
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Observe and report the presence of pressure areas and skin breakdowns to prevent bedsores.
  • Report injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents and record/report data.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Honor resident's refusal of treatment requests and report them to a supervisor.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
