About The Position

Responsible for attending to patient medical needs and assisting in the maintenance of a safe therapeutic environment.

Requirements

  • Must possess high school diploma or GED equivalent.
  • Must possess and maintain active Certified Nursing Assistant Certification or Nursing Assistant Registered in Washington State from accredited program.
  • One (1) year prior experience as a CNA/NAR required.
  • Must possess a valid WA state driver’s license.
  • Demonstrated proficiency in the use of Microsoft Office and the ability to use various software programs.
  • Must be able to successfully pass a comprehensive criminal background check (Federal and state).
  • AFH Staff: ability to successfully pass the Department of Social and Health Services background check.
  • For positions involving driving personal vehicles or company vehicles as part of the job duties, valid drivers license, proof of personal liability insurance.
  • AFH or other assigned positions – successfully pass THIRA’s Driver Assessment.
  • Ability to work evenings, nights, weekends & holidays as needed.
  • Perform other duties as assigned.

Responsibilities

  • Assist THIRA nurse to create, maintain, and track patient health history and medical documentation in electronic medical record systems.
  • Take vitals on patients daily and/or as needed; document accordingly in patient's chart.
  • Perform initial new patient medical assessment, including but not limited to, reconcile medications, obtain medical/surgical history and most recent labs, confirm all patient allergies, and document all findings in patient chart.
  • Maintain thorough medical/medication data for patients in EMR patient charts.
  • Observe and report changes in patient’s behaviors, physical symptoms, and mental health status to the Medical Team.
  • Perform skin check on patients with a history of Non-Suicidal Self Injury (NSSI) and inform the team of any observed evidence of recent NSSI.
  • Assist patients with self-administered medication while on-site at program; document accordingly.
  • Administer lab and genetic testing orders and enter prescription refill requests as needed.
  • Provide meal and snack supervision; document as needed.
  • Support and assist psychotherapists, dietitians, art therapist, and milieu support therapist in creating and maintaining a safe and therapeutic milieu environment.
  • Maintain infection control standards within our facility, ensure employees and patients uphold procedures and standards as relevant.
  • Obtain urine specimens and send to lab for random patient drug screening, follow up as necessary.
  • Communicate with outside providers and patient family members regarding medical information as needed.
  • Support maintenance of patient illness tracking log and consult with patients regarding questions/concerns about presenting physical medical symptoms.
  • Provide monthly audits of over-the-counter medications, emergency medications, and first aid kits to ensure that expired medications are disposed of and replaced.
  • Ensure maintenance of medical info binders for all required patients to be used for emergency situations.
  • Clean, sanitize and maintain safe nurse’s station.
  • Assist in coordination for the disposal of biohazard waste with appropriate vendors.
  • Assist with inventory and maintenance of medical supplies.
  • Serve as point of contact for new patients entering the program orienting them to program structure, space and rules.
  • Participate in activities with residents at the house; house meeting, cooking, games.
  • Safely drive/transport patients to and from the Retreat at THIRA and facility.
  • Accompany residents and therapeutic staff on recreational and therapeutic outings and ensure patient safety.
  • Complete rounds on patients at minimum of 1-hour time intervals, more frequently if needed as clinically indicated. Document in daily review.
  • Support Compliance Department by conducting chart audits and relaying findings to Senior Compliance Coordinator.
  • Facilitate start to program day and ensuring patients engage in designated morning hygiene time appropriately.
  • Facilitate morning medication pass.
  • Set up breakfast and hand off to Dayshift.
  • Support and maintain policies regarding professional ethics, including appropriate boundaries and client confidentiality; monitor documents according to company and HIPAA policies.
  • Report any immediate safety hazards to appropriate supervisory personnel or maintenance personnel.
  • Adhere to WA State Requirements for Mandated Reporting.
  • Demonstrate awareness and respect for diverse clients and staff.
  • Adhere to THIRA Health’s compliance standards, Ethics and Code of Conduct Policy and Conflict of Interest Policy.
  • Attend and actively participate in all required THIRA meetings, trainings, and all other coverage, duties, and special projects as assigned.
  • Exercise discretion and judgement with sensitive and confidential matters, including PII, PHI, and HIPAA requirements.
  • Possess strong verbal and written English communication skills.
  • Demonstrated ability to multi-task.
  • Demonstrated ability to work in a fast-paced environment with limited supervision.

  • Effectively communicate with all levels of staff, visitors, healthcare professionals, marketers, and the public in a professional, sensitive, and interpersonal manner.
