About The Position

Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years. We serve all of San Joaquin and Stanislaus Counties. We are seeking a Dynamic, self-motivated CNA - Per Diem who thrives in a fast-paced environment and supports our mission. POSITION PURPOSE/SUMMARY: As a member of the multi-disciplinary team the CNA will provide personal care services to the terminally ill patient/family in accordance with a written plan of care.

Requirements

Good verbal and written communication skills are required.
Demonstrated ability to make meaningful observations and write a brief, accurate reports.
Demonstrated ability to recognize the needs of people and to maintain good relationships in dealing with them.
Experience as health aide/CNA.
Ability to deal effectively with stress.
High school graduate or equivalent degree.
Flexibility.

Responsibilities

Services are carried out according to Hospice of San Joaquin's policies.
Provide or assist the patient in bathing, and care of mouth, skin, or hair.
Assist patients in and out of bed and assist with ambulation.
Turn patients in bed and positions for comfort.
Assist with prescribed exercises which patients and Health Aide/CNA have been taught by appropriate health personnel.
Prepare and serve meals and assist patients with eating.
Assist patients to the bathroom or in using a bedpan or urinal.
Give enemas as ordered and instructed.
Perform household services that will facilitate the patient’s self-care at home as necessary to prevent or postpone institutionalization.
Change bed linens.
Take and record vital signs as directed.
Complete appropriate records.
Adheres to all Hospice of San Joaquin policies, and procedures, including following Hospice of San Joaquin infection control procedures and the code of ethics.
Other assignments may be appropriate.
