About The Position

Sanford Health, the largest rural health system in the United States, is dedicated to transforming the health care experience and providing access to world-class health care in America’s heartland. This position provides resident-centered personal care including bathing, toileting, grooming, dressing/undressing, obtaining vital signs, psychosocial support and other personal cares. The role involves assisting residents with transfers, repositioning, and walking using appropriate techniques and equipment, as well as providing range of motion and passive exercise. Responsibilities also include tasks related to dining and food preparation, assisting residents with meals and snacks, providing water and supplements, and ensuring a positive dining experience. The position requires accurate medication administration and documentation of outcomes related to resident cares, medications/treatments, behaviors, activities, and dietary intake. Housekeeping and laundry services are provided to maintain clean living areas, with a focus on hand washing and Personal Protective Equipment (PPE) use. Assistance and documentation with wellness and recreational programs are also part of the role, encouraging resident participation. A desire to work with residents in sub-acute care and long-term care is essential.

Requirements

High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16.
Prior clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement.
CNA training and certification required and provided by facility.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year.
A minimum of 12 hours continuing education are required annually.
Maintains all department specific required certifications.
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license.
Also must be medically qualified to perform the essential driving functions of this position as per our Sanford Driving Policy per the leaders request.

Responsibilities

Accurately documents outcomes related to resident cares, medications/treatments, behaviors, activities and dietary.
Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational; follows consistent hand washing and Personal Protective Equipment use.
Provides assistance/documentation with wellness and recreational programs and encourages residents to participate.
