About The Position

Tryfacta is seeking a CNA / Nurse Assistant for our client in Vestal, NY 13850. This is a 16-week contract assignment. The role involves providing direct patient care to residents in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) setting. Responsibilities include assisting residents with daily living activities, monitoring their condition, documenting vital signs, and supporting residents with chronic conditions while promoting dignity and independence. The position also requires adherence to infection control procedures, safety regulations, and assisting with resident admissions, transfers, and discharges.

Requirements

• Active New York CNA License required (license must be active with no license hits).
• Minimum 1 year of Long-Term Care (LTC) experience required (SNF, Rehab, Med-Surg, or Hospital experience also accepted).
• BLS Certification required.
• Must be comfortable working Mid 5x8-Hour (14:45 - 23:15)

Nice To Haves

• Experience caring for Alzheimer's and Dementia residents preferred.

Responsibilities

• Provide direct patient care to residents in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) setting.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, toileting, transferring, and mobility.
• Monitor and report changes in residents' physical and emotional condition to the licensed nursing staff.
• Obtain and document vital signs, weights, and intake/output as assigned.
• Maintain accurate and timely documentation in accordance with facility policies.
• Support residents with Alzheimer's disease, dementia, and other chronic conditions while promoting dignity and independence.
• Follow infection control procedures and safety regulations at all times.
• Assist with admissions, transfers, and discharge preparation.
