Responsibilities
- Assist residents with activities of daily living (ADLs) , including bathing, dressing, grooming, toileting, and feeding.
- Take and record vital signs , including temperature, pulse, respiration, and blood pressure.
- Assist residents with transfers, ambulation, positioning, and mobility using appropriate equipment.
- Provide personal care and comfort while maintaining residents' dignity and privacy.
- Assist with meals, nutrition, and hydration as needed.
- Observe residents for changes in condition and report concerns to the nurse promptly.
- Provide incontinence and toileting care and maintain proper hygiene.
- Follow infection-control, safety, and facility procedures .
- Document care provided accurately in the facility's charting/EMR system .
- Support residents with range-of-motion and restorative care activities as directed.
- Maintain a clean, safe, and comfortable resident environment.
- Communicate professionally with residents, families, nurses, and other healthcare team members
