About The Position

Tryfacta is seeking a CNA LTC for our client in Crivitz, WI 54114. This is a 13-week contract assignment. The role involves providing compassionate and hands-on care to residents in a long-term care setting, assisting with daily living activities, monitoring resident conditions, and ensuring resident safety. The facility is a 43-bed unit with two halls, serving rehabilitation, short-stay, LTC, and dementia-care residents.

Requirements

• Current CNA certification.
• Strong communication, teamwork, and resident-care skills.
• Ability to work evening shifts as scheduled.

Nice To Haves

• Long-term care/SNF experience preferred.
• Experience caring for dementia residents is preferred.
• MatrixCare charting experience is preferred.

Responsibilities

• Provide compassionate and hands-on care to residents in a long-term care setting.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
• Monitor and report changes in residents' condition to the nursing team.
• Assist with transfers, repositioning, feeding, and maintaining resident safety.
• Provide care for rehabilitation, short-stay, LTC, and dementia-care residents.
• Complete accurate resident documentation and charting using MatrixCare.
• Maintain a clean, safe, and comfortable environment for residents.
• Work collaboratively with RNs, LPNs, rehabilitation staff, and other healthcare professionals.
