About The Position

Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, founded in 1982 as Joliet Area Community Hospice. They provide exceptional serious illness care, hospice care, and grief support for adults and children. The majority of their direct patient care is in the field, with team members providing care in homes, nursing facilities, or assisted living facilities. They also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of their Joliet administrative offices. As a member of the dynamic Inpatient Unit team, the CNA will provide quality patient care to patients and assist them with their ADL’s. Prior experience in a hospital setting, nursing home, or long-term care is preferred. The Inpatient Unit team works 12-hour shifts in the facility, with the position working three (3) shifts a week from 7pm – 7:30am with rotating weekends and holidays.

Requirements

  • High school diploma or GED
  • Meets the training requirements of Illinois
  • Current CNA certification

Nice To Haves

  • Prior experience in hospital setting, nursing home or long-term care preferred

Responsibilities

  • Provide compassionate care, emotional support, and companionship to patients and families
  • Assist with personal care, mobility, and daily living activities
  • Monitor, document, and report patient condition and changes to the RN
  • Ensure patient safety and proper use of equipment
  • Maintain a clean, safe, and comfortable home environment
  • Follow care plans, policies, and participate in training and team activities
