Corewell Health Hospice specializes in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less. They serve individuals in their homes, nursing facilities, and hospitals. In collaboration with the interdisciplinary team, the CNA assesses, plans, implements, coordinates, monitors, and evaluates strategies and services to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted using the nursing process, relationship-centered care principles, and a holistic philosophy to ensure the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, and sacred death.
