About The Position

Coverage Territory: Effingham, Mattoon and surrounding areas Schedule: Mon-Fri (8 hour shifts) Be There When It Matters Most At Transitions Hospice, we don't just manage care — we protect dignity, comfort, and the moments that matter most. We've reimagined hospice to be something different: compassionate, unhurried, and built around the whole person. That means someone is always present — through the quiet hours of the night — so patients feel safe, families can rest, and no one faces the darkness alone.
