About The Position

The Certified Nursing Assistant is responsible for providing optimal patient care within the home, under the direction of an RN. This role involves direct patient care, maintaining a safe environment, promoting patient independence, and comprehensive documentation.

Requirements

  • Must be certified as a nurse assistant or home health aide.
  • Any lapse in providing services in this capacity must be less than 24 months in duration.
  • High School graduate or G.E.D.
  • Current CPR certification.
  • Able to provide own transportation to patients’ homes.
  • Positive mental and physical health.
  • Strong interpersonal skills and communication skills.
  • Satisfactory references.
  • Able to work beyond scheduled hours as needed.

Nice To Haves

  • One year experience in hospital, nursing home, or home care desired.
  • Home health care preferred.

Responsibilities

  • Provides direct care and personal care services to patients in the home environment, including bathing, dressing, oral hygiene, hair care, nail care, shaving, changing bed linens, and skin care.
  • Assists with patient's elimination needs, including use of bed pans, urinals, and commodes, or assisting to the bathroom.
  • Performs procedures such as vital signs, weight, catheter care, urine testing, assisting with colostomy bag changes and reinforcement of dressings, and reminding patients to take medications as instructed by the RN.
  • Assists with walking, standing, sitting, transferring, and positioning, with or without assistive devices.
  • Assists with prescribed range of motion exercises as taught by agency healthcare professionals.
  • Prepares special diets and assists with feeding.
  • Measures intake and output of fluids.
  • Immediately reports any changes in patient's condition, appearance, or behavior, or any problems carrying out assigned duties to the HomeCare RN or therapist supervisor.
  • Performs light housekeeping tasks such as patient's personal laundry, vacuuming, sweeping, and dusting of the patient's immediate environment and tidying the bathroom after bathing patient.
  • Straightens the patient's room as necessary.
  • Attempts to remedy and instruct the patient/family regarding apparent safety hazards.
  • Promotes patient independence and optimal physical and emotional functioning through activities such as reading, conversation, assisting with correspondence and other socialization.
  • Assists with grocery shopping and accompanying patient to appointments.
  • Supervises and assists with self-care activities, encouraging mobility and activity to the extent permitted.
  • Keeps professional staff informed of the patient’s mental and emotional status.
  • Provides comprehensive documentation in the patient’s medical record, recording all services rendered and patient responses to care in accordance with policy and regulatory requirements.
  • Completes all patient records and agency forms accurately and in a timely manner.
  • Organizes work to adequately perform patient care duties as assigned.
  • Collaborates with management to identify issues requiring further exploration.
  • Participates in the agency's Quality Improvement Program.
  • Participates in agency meetings, activities, programs, inservices, and continuing education classes.
  • Continues to develop skills and knowledge base to assist in providing quality patient care.
  • Remains flexible to changes in agency policy and procedure and in daily scheduling.
  • Maintains collegial relationships with other disciplines, interpreting the role and responsibilities of the HomeCare Aide to others.
  • Communicates and interprets agency policies and procedures to team members, patients, and families.
  • Communicates with management if policies/procedures are violated.
  • Follows agency policy and procedures for emergencies.
  • Works tactfully and cooperatively with patients, family members, and staff.
  • Assists patients, families, co-workers, and other health care members to resolve expressed concerns.
  • Minimizes infectious diseases by following infection control policies and protocols.
  • Attends ongoing inservices education programs regarding safety issues, equipment, and HIV precautions.
  • Keeps information confidential by adhering to human resources policy concerning confidential information.
  • Demonstrates appropriateness in meeting objectives in age-specifics.
  • Performs other related duties as may be required.
