About The Position

Responsible for assisting the patient with activities of daily living and personal care. The C.N.A., in collaboration with patient, family, and health care team, will provide direct and indirect nursing care of the patient utilizing the nursing process under the supervision of a Registered Nurse according to the following work systems/functions. Requirements

  • High school graduate or G.E.D.
  • Must possess current licensure as required by state board where services will be provided: MD: Certified Nursing Assistant through the Maryland Board of Nursing OH: Certified Nursing Assistant through the Ohio Department of Health PA: Certified Nursing Assistant through the Pennsylvania Department of State WV: Certified Nursing Assistant through the West Virginia Department of Health & Human Resources
  • Valid driver’s license.
  • CPR certification within 30 days of date of hire.

Responsibilities

  • Provides personal and hygienic care every visit as per care plan and basic care standard in accordance with age-specific needs and policy and procedure.
  • Maintains a clean, safe, and comfortable environment per the basic care standard within the limitations of the home setting.
  • Ensures privacy and demonstrates concern for dignity of patient and respects patient’s property in performing all patient care.
  • Completes assignments in a timely manner as per standard.
  • Follows plan of care assigned by RN.
  • Relays critical information to the appropriate staff regarding patient condition.
  • Confers with the primary nurse on the original plan of care and any subsequent updates.
  • Documents patient status and care of services furnished in appropriate areas as defined by the documentation policy and procedure.
  • Reports observed changes in patient condition to R.N.’s, including recognition and reporting of changes in level of comfort.
  • Reports abnormalities in vital signs to R.N.
  • Utilizes supplies effectively and to ensure cost containment.
  • Perform the following treatments/procedures according to the specific standards included but not limited to: Measuring vital signs, Lifting and moving patients, Positioning the patient in bed, Transferring patient from bed to chair, Ambulating patient, ROM, Bed-making, Personal Hygiene (Oral care, Bathing, Back massage, Perineal Care, Hair Care, Shaving, Nail Care, Changing clothes), Assisting with elimination (Bed pan, Urinal, BSC, Ostomy care), Patients’ weights, Case care.
  • Identifies the stages of death and dying.
