About The Position
This position provides direct patient services that are physician ordered. The Home Health/Hospice Aide will provide hands-on personal care and support services under the direction and supervision of a RN. This role will assist patients with ambulation and safe transfers per agency policies, report and document condition changes to the Case Manager, and participate in care coordination meetings per company policy. The role requires the ability to lift up to 50 lbs and assist with patient transfers, respect patients' homes and environments, and maintain required continuing education for a CNA. This is a Home Health/Hospice Aide position and is not related to Dialysis.
Requirements
- Current CNA license
- Current Drivers License
- 1 year experience in Home Health, nursing home or hospital setting.
- Must be computer literate and be able to document electronically.
- Must have current CPR certificate
- Must be able to pass State required onboarding testing.
- Must have reliable transportation and current auto insurance

- Must keep up required continuing education requirements for a CNA.
