Under the supervision of the Nurse and Homecare Manager, a CNA-Home Care provides personal care and homemaker services according to individual care plans for participants of the PACE Organization of Rhode Island. Oversight of performance and job duties is conducted by a Nurse during observation visits and more frequently as needed. Keeps current on and follows individual participant care plans. Consistently “clocks in & out” of participants’ homes and documents completed ADL/IADLs in AxisCare. Provides personal care and services as assigned including toileting, bathing/showering, eating assistance, grooming and oral care. Provides light homemaking tasks as assigned including meal prep, shopping and laundry services. Ensures a clean, sanitary, and safe environment is maintained. Provides behavioral supervision and support for participants, monitors for and responds to behavioral disturbances such as wandering, agitation, restlessness, exit seeking, etc. Observes participants for emotional, behavioral, and physical changes in condition including, but not limited to any changes in mood, skin integrity, bowel and bladder status, food/fluid intake, speech, ambulation and transfer ability. Promptly reports participant change of condition to Home Care Department. Responds appropriately and in a timely manner to personal alarm devices if present in the home. Adheres to PACE's Participant Rights Policy at all times. Adheres to PACE safety requirements and participant care plans regarding use of assistive devices (e.g., gait belts, hoyer lifts, slide boards, etc.). Uses proper universal and standard precautions and COVID-19 PPE protocol according to PACE policy and procedure. Assists with implementing recreation programs in a home setting. Assists participants with physical activities in support of restorative care plans (e.g. walking for exercise) as assigned and trained by the rehab department to implement in the home setting. Notifies Home Care Department of low inventory to avoid running out of supplies. Attends staff and participant care planning meetings as requested, and department and agency in-services as required. Protects privacy and maintains confidentiality of sensitive participant and agency information. Works in PACE-RI Day Centers as assigned. Works rotating weekends and/or holidays as assigned. Performs related duties as assigned.
