Responsibilities
- Provide hands-on support based on each patient’s individual needs and care plan.
- Assisting patients with personal care and daily living
- Supporting mobility and safe transfers as needed
- Providing companionship and reassurance
- Observing and reporting changes in the patient’s condition
- Maintaining accurate documentation
- Following the patient’s care plan and agency policies
