Under the supervision of the Day Center Director, this role provides personal care and services according to individual care plans for participants of the PACE Organization of Rhode Island. As a member of the Interdisciplinary Team (IDT), the CNA reports regularly on personal care services provided, participants' self-care abilities, and their expressed wishes. The role involves attending care plan meetings as requested and staying current with and following individual participant care plans. Adherence to safety requirements and care plans regarding the use of assistive devices is crucial. The CNA will provide direct personal care, monitor vital signs, offer behavioral supervision and support, and respond to personal alarm devices. Assistance with physical activities for restorative care, adherence to universal precautions and COVID-19 PPE, and following nursing instructions are key duties. Accurate record-keeping, assisting with meal service and dining area cleanup, and maintaining the order of personal care areas are also responsibilities. The role includes assisting with laundry, implementing recreation programs, and collaborating with other departments to ensure participant needs are met and a safe environment is maintained. May be required to provide in-home care, escort participants to appointments, and adhere to PACE policies and procedures, including participant rights and confidentiality. Attendance at staff and participant care planning meetings and department/agency in-services is required. The position involves rotating weekends and/or holidays as assigned and may require working at other PACE-RI sites or performing other duties as assigned.
