About the Role: The (CNA) Certified Nursing Assistant plays a vital role in providing compassionate, personalized care to patients in their homes across the United States. This position focuses on assisting individuals with daily living activities, ensuring their comfort, safety, and well-being while promoting independence. The Certified Nursing Assistant CNA collaborates closely with healthcare professionals to implement care plans tailored to each patient's unique needs. Through attentive observation and communication, the role contributes to monitoring patients' health status and reporting any changes promptly. Ultimately, this position aims to enhance the quality of life for patients by delivering respectful, empathetic, and professional home health care services.

Requirements

  • Valid CNA certification recognized in the State of New Jersey
  • High school diploma or equivalent.
  • Ability to pass background checks and health screenings as required by state regulations.
  • Demonstrated ability to perform physical tasks associated with patient care.
  • Strong communication skills and basic literacy to document care activities accurately.

Nice To Haves

  • Previous experience working in nursing home or sub acute environment.
  • CPR and First Aid certification.
  • Familiarity with electronic health record (EHR) systems.
  • Additional training or certification in specialized care areas such as dementia or palliative care.
  • Bilingual abilities to effectively communicate with diverse patient populations.

Responsibilities

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
  • Provide basic health-related services including monitoring vital signs and reporting any changes to nursing staff.
  • Support patients with medication reminders and adherence under the supervision of healthcare professionals.
  • Maintain a clean, safe, and comfortable environment for patients within their homes.
  • Document care provided and communicate effectively with the healthcare team to ensure continuity of care.
  • Offer emotional support and companionship to patients to promote mental and emotional well-being.
  • Adhere to all safety protocols, infection control standards, and patient confidentiality regulations.
