About The Position

Under the direction and supervision of a licensed nurse, performs basic nursing care procedures. This role involves assisting residents/patients with daily living activities, monitoring their condition, documenting care, and adhering to facility policies and procedures. The position supports the psychosocial needs of residents and requires participation in educational programs to maintain updated knowledge and skills.

Requirements

  • State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.
  • Ability to interact professionally with coworkers.
  • Ability to prioritize tasks.
  • Function as a team member.
  • Ability to speak, read English in order to communicate with patients, residents and clinical staff.
  • Provides care and services that demonstrate behaviors and the principles of person-centered care including affording people dignity, respect and compassion, offering coordinated care, support and treatment and care that is personalized.

Nice To Haves

  • High School diploma or GED preferred
  • 6 months – 1 year experience as a CNA preferred

Responsibilities

  • Performs basic nursing care procedures under the direction and supervision of a licensed nurse, including assisting with meals, transfers, ambulation, bathing, toileting, dressing, and grooming.
  • Observes and communicates pertinent information about the resident/patient's condition to appropriate facility staff.
  • Takes and records pulse, respirations, blood pressures, and weight as needed.
  • Documents care and observations on worksheets and EMR accurately and in a timely manner.
  • Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
  • Adheres to infection control practices.
  • Supports the psychosocial needs of the residents as per the plan of care and profile.
  • Participates in educational and inservice meetings to update knowledge of techniques, guidelines, and regulations, including attending all mandatory programs and meeting the basic requirement for 12 hours of continuing education per year.
  • Uses equipment in an appropriate and safe manner and adheres to standards of the Diligent Minimal Lift Program.
  • Follows facility policies and procedures.
  • Performs other duties as required.
