About The Position

MAB Community Services is a non-profit human service organization serving adults with developmental disabilities, and adults with acquired brain injuries. We have an exciting opportunity for a detail-oriented, candidate who enjoys assisting individuals to reach their full potential. The Case Manager role will provide a rewarding opportunity to work with individuals in skilled nursing facilities to assist them with a transition to the community and ongoing community supports. The Case Manager role requires proven administrative skills, ability to submit documentation in a timely and efficient manner, proficiency with case management tasks.

Requirements

• Must have excellent written and oral communication skills, problem solving skills, and computer literacy.
• A minimum of two years experience working with adults with disabilities, mental health, and/or Brain Injuries is required.
• Valid driver's license, clean driving record, and reliable transportation required.
• Updated CPR and First Aid Certifications required, or ability to become certified immediately upon hire.

• A bachelor's degree in a related field is preferred.

Responsibilities

• Housing search and housing applications
• Attending housing appointments
• Applying for documents, such as an ID or Social Security Card
• Coordinating additional assessments, such as PT or OT
• Purchase of furniture and essential items
• Setting up utilities and purchased items in new homes
• Coordinating medical appointments
• Assisting with tasks in the community
• Coordinating Transportation
