About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct personalized health evaluations at patients' homes, offering a convenient and comprehensive healthcare experience. The visits, lasting approximately 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The role also includes conducting virtual visits as needed and providing all Signify Health services, including Diagnostic & Preventive Services products like spirometry and diabetic eye exams.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification (required).

Nice To Haves

Multi-state licenses (preferred).
Able to cover the following primary locations: Denver MSA Denver County.

Be flexible to travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
