The CTT Clinician’s role is to provide clinical interventions and support to youth and families as youth transition from residential placement to the community. The Clinician’s role is a time-limited clinical intervention intended to bridge the youth’s transition to the community following a residential stay. CTT clinicians work in conjunction with the residential treatment team to support this transition and are available to youth and families to effectuate successful clinical supports throughout this transition when community-based services are inaccessible. The overarching goal of CTT is to either terminate when short-term treatment goals have been met or when the youth/family has successfully transitioned to a sustainable clinical resource in the community. Youth in the community may be considered for referral to CTT if they need immediate clinical support and are having difficulty connecting with a community provider.
