About The Position

Generations Family Health Center is seeking a licensed Clinical Therapist to join their team in Putnam, CT. This role involves providing comprehensive behavioral healthcare services to patients of all ages within a regional community health center. Requirements

  • Master’s degree in social work or related human service field.
  • Experience working with individuals, couples and families.
  • Licensed candidates preferred (LCSW/LMFT/LPC/LMSW/LMFTA/LPCA).
  • Innovative self-starter and strong organizational skills desired.
  • Experience with patients across the lifespan, including under 18, preferred.
  • Bilingual candidates (Spanish/English) preferred based on patient population.

Nice To Haves

  • Experience with Substance Use Treatment is preferred but not a requirement.

Responsibilities

  • Conduct clinical evaluations and assessments, diagnose and provide treatment for patients.
  • Conduct individual, group, family and/or couples therapy sessions for patients across the lifespan in accordance with accepted American Psychiatric Association guidelines for treatment techniques and therapies.
  • Develop and coordinate a clinical treatment program for the diagnosis, treatment, or referral of patients presenting with behavioral health needs.
  • Provide a multi-disciplinary approach to treatment plan, coordinating with medical providers and/or other behavioral health clinicians.
  • Assess patient progress and modify treatment program accordingly.
  • Coordinate care of patient with referring physician or agencies.
  • Document all therapies, according to established protocols.
  • Attend required meetings and participate in committees as requested.
  • Participate in professional development and maintain required continuing education hours for licensing.
  • Maintain patient confidentiality and document case records on all contacts regarding patient.
  • Opportunities to develop and facilitate therapy groups.
  • Consistent supervisory meetings with Clinical Supervisor.
  • Partner with Medical Department on patient care.
  • Opportunities to supervise clinician interns.
