About The Position

Generations Family Health Center is seeking a licensed Clinical Therapist to join their team in Putnam, CT. This role involves providing comprehensive behavioral healthcare services to patients of all ages within a regional community health center. Requirements

Master’s degree in social work or related human service field.
Experience working with individuals, couples and families.
Licensed candidates preferred (LCSW/LMFT/LPC/LMSW/LMFTA/LPCA).
Innovative self-starter and strong organizational skills desired.
Experience with patients across the lifespan, including under 18, preferred.
Bilingual candidates (Spanish/English) preferred based on patient population.

Nice To Haves

Experience with Substance Use Treatment is preferred but not a requirement.

Responsibilities

Conduct clinical evaluations and assessments, diagnose and provide treatment for patients.
Conduct individual, group, family and/or couples therapy sessions for patients across the lifespan in accordance with accepted American Psychiatric Association guidelines for treatment techniques and therapies.
Develop and coordinate a clinical treatment program for the diagnosis, treatment, or referral of patients presenting with behavioral health needs.
Provide a multi-disciplinary approach to treatment plan, coordinating with medical providers and/or other behavioral health clinicians.
Assess patient progress and modify treatment program accordingly.
Coordinate care of patient with referring physician or agencies.
Document all therapies, according to established protocols.
Attend required meetings and participate in committees as requested.
Participate in professional development and maintain required continuing education hours for licensing.
Maintain patient confidentiality and document case records on all contacts regarding patient.
Opportunities to develop and facilitate therapy groups.
Consistent supervisory meetings with Clinical Supervisor.
Partner with Medical Department on patient care.
Opportunities to supervise clinician interns.
