This role is responsible for supervising clinical staff, ensuring care is delivered safely, effectively, and in compliance with agency, state, federal, and accreditation standards. The Clinical Supervisor serves as a key operational and clinical resource supporting patient outcomes, staff performance, care coordination, and day-to-day workflow management across the Home Health and mobile care continuum.
Requirements
- Current Registered Nurse (RN) license in the practicing state
- Current CPR certification
- Minimum of 2 years of Home Health experience
- Strong knowledge of Home Health regulations, documentation, and patient care coordination
Nice To Haves
- 2+ years of clinical leadership, supervisory, or management experience
- Experience supervising field-based or mobile care clinicians
- Knowledge of OASIS documentation and Medicare compliance
Responsibilities
- Provide clinical supervision and coordination of Home Health patient care services
- Ensure care delivery aligns with physician orders, individualized care plans, and agency policies
- Supervise and support field clinicians to maintain competency, compliance, and quality patient care
- Provide coaching, mentorship, and performance feedback to clinical staff
- Serve as a clinical resource for escalated patient, caregiver, and staff concerns
- Utilize electronic medical records (EMR) to coordinate and monitor patient care services
- Review clinical documentation, including OASIS and visit notes, for accuracy, completeness, and timeliness
- Support coordination of referrals, admissions, transfers, and discharges
- Collaborate with interdisciplinary teams to ensure seamless patient transitions and continuity of care
- Help ensure compliance with Medicare Conditions of Participation and regulatory requirements
- Participate in quality improvement initiatives, audits, and corrective action plans
- Promote adherence to HIPAA, agency standards, and patient safety protocols
- Support operational excellence and continuous improvement within Home Health services
