This role is responsible for supervising clinical staff, ensuring care is delivered safely, effectively, and in compliance with agency, state, federal, and accreditation standards. The Clinical Supervisor serves as a key operational and clinical resource supporting patient outcomes, staff performance, care coordination, and day-to-day workflow management across the Home Health and mobile care continuum.

Requirements

  • Current Registered Nurse (RN) license in the practicing state
  • Current CPR certification
  • Minimum of 2 years of Home Health experience
  • Strong knowledge of Home Health regulations, documentation, and patient care coordination

Nice To Haves

  • 2+ years of clinical leadership, supervisory, or management experience
  • Experience supervising field-based or mobile care clinicians
  • Knowledge of OASIS documentation and Medicare compliance

Responsibilities

  • Provide clinical supervision and coordination of Home Health patient care services
  • Ensure care delivery aligns with physician orders, individualized care plans, and agency policies
  • Supervise and support field clinicians to maintain competency, compliance, and quality patient care
  • Provide coaching, mentorship, and performance feedback to clinical staff
  • Serve as a clinical resource for escalated patient, caregiver, and staff concerns
  • Utilize electronic medical records (EMR) to coordinate and monitor patient care services
  • Review clinical documentation, including OASIS and visit notes, for accuracy, completeness, and timeliness
  • Support coordination of referrals, admissions, transfers, and discharges
  • Collaborate with interdisciplinary teams to ensure seamless patient transitions and continuity of care
  • Help ensure compliance with Medicare Conditions of Participation and regulatory requirements
  • Participate in quality improvement initiatives, audits, and corrective action plans
  • Promote adherence to HIPAA, agency standards, and patient safety protocols
  • Support operational excellence and continuous improvement within Home Health services
