In this leadership role, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation ensuring support is easily accessible in a timely fashion; You will participate in the identification, development, design, and implementation of nursing educational programs to support clinical excellence and determine success measures and evaluate the effectiveness of these programs to promote ongoing staff development as well as patient and customer satisfaction. You will also meet regularly with our customer to maintain relationships and provide updates such as readmission reports, offering insights' to improve patient outcomes and reduce readmissions. This is a hybrid of field work and on site based at 21HHS office: Office : Spend 2 days per week in the office for administrative and team management duties. Fieldwork : Spend 3 days per week (average 12+ per week) in the field conducting supervisory visits and offering direct support to RN Case Managers
