About The Position

Heritage Senior Living is hiring immediately for a Clinical Supervisor | LPN Supervisor. As a leader in senior care and services throughout Wisconsin, Heritage Senior Living is looking to hire an experienced Nurse to join our Clinical and Quality Specialist Nurse corporate support team. Our goals are to enhance residents’ quality of life and to continue our growth and success, with plans to open new senior communities annually and develop centers of excellence for comfort, care, quality, and specialized dementia services. The Quality and Clinical Specialists ensure HSL policies and standards are practiced, state regulations are followed, and infection prevention strategies are implemented.

Requirements

  • Valid state of Wisconsin RN licensure
  • Must be in good standing on the State Board of Regulation and Licensing
  • Three years of nursing experience working with seniors
  • Must be self-directed and able to function independently
  • Excellent interpersonal skills – ability to connect with residents, families, and associates
  • Strong customer service orientation to older adults, exhibit a caring and compassionate attitude
  • Ability to make responsible choices and decisions that are in the best interest of the residents and caregivers
  • Ability to work collaboratively with all coworkers
  • Excellent written and verbal communication skills
  • Must be proficient in Microsoft
  • Must submit to all applicable pre-employment and annual tests per state and federal regulations
  • Must exhibit a caring and compassionate attitude while articulating true concern for people

Nice To Haves

  • experience within an assisted living, memory care or other long-term care environment preferred
  • Managerial experience preferred
  • PointClickCare software experience a plus
  • Ability to problem solve and communicate complex issues

Responsibilities

  • Supports adherence to HSL policies and procedures and provides guidance to communities on policy implementation.
  • Assists with policy and procedure development and modifications as needed.
  • Educates staff, managers, and leaders of assigned communities on quality improvement processes.
  • Works directly with assigned community leadership teams to monitor indicators, dashboards, and performance improvement projects.
  • Tracks, analyzes, and reports on all PIPs to the Quality team.
  • Assists assigned communities with implementing, monitoring, and tracking resident, family, and employee satisfaction surveys.
  • Implements and monitors community action plans.
  • Facilitates performance initiatives at assigned communities and assists with plans of correction as needed.
  • Completes quality indicator tracking and trending reports.
  • Supports community leaders on quality initiatives.
  • Monitors compliance in accordance with Heritage Senior Living policies, best practice protocols, and state and federal regulatory requirements.
  • Provides education to staff on the electronic health record system to ensure compliance with documentation standards.
  • Participates in and monitors new company programs and pilots as necessary.
  • Acts as interim DON depending on the needs of the organization.
  • Manages the training and onboarding of DONs.
  • Assists with Corporate Onboarding, which includes orientation & CBRF trainings.
  • Partners with the Vice President of Clinical & Quality Operations to develop, implement, and evaluate annual infection prevention goals and action plans.
  • Partners with facility leaders, physicians, local, state, and national agencies on activities related to infection prevention.
  • Oversees the operations of the infection prevention, epidemiology, industrial hygiene, and relevant safety programs.
  • Accountable for surveillance of healthcare acquired and community acquired infections.
  • Ensures that education and counseling on infection prevention is available for staff, volunteers, medical staff, patients, and parents.
