About The Position

UW Health Care Direct has been an affiliate of UW Health since 1994. In July of 2020 UW Health Care Direct (formerly known as Chartwell Midwest Wisconsin) officially consolidated all community- based home health services to Chartwell from UW Health and Unity Point Health leading to the establishment of UW Health Care Direct. UW Health Care Direct is dedicated to providing home health care services in the Dane County area, ensuring the highest quality of care for the community.

Requirements

State of Wisconsin Professional licensure for Pharmacist
Bachelor’s degree in: Pharmacy
PharmD
Individuals who are pharmacist vaccinators are required to be CPR certified.
Board certification in nutrition support required within the first year of employment.
Participation in weeknight and weekend on call shifts is required.

Nice To Haves

Completion of a PGY-1 hospital residency or minimum of 2 years of recent experience in an acute care setting or one year of recent experience in home infusion therapy preferred.
Experience and strong knowledge base in intravenous admixture services and intravenous drug therapy preferred.

Responsibilities

Coordinates and facilitates care of new and on service patients’ infusion needs through collaboration with prescribers and health system staff along with Care Direct team members and operations.
Practices the pharmacists’ patient care process in providing safe and effective drug therapy.
Provides clinical and legal review of all new patient prescriptions. Maintains pharmacy records in accordance with applicable laws and regulations.
Collects and assesses relevant information to develop patient focused plan including medication appropriateness, medication reconciliation, and monitoring parameters including laboratory and clinical data. Provides recommendations to prescriber for plan alterations when appropriate to enhance outcomes associated with drug therapy.
Provides medication consultations to patients and dispenses all medications and related supplies and equipment in accordance with federal and state laws and standards of practice which govern the profession of pharmacy and sterile compounding.
Checks all medications, supplies, and equipment prior to dispensing from the pharmacy.
Completes thorough and timely documentation of patient notes and charting in the organization’s electronic health record.
Manages patient care as delegated by a physician through any written delegation agreements.
Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and/or conferences.
Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
Focuses on improving processes and champions efficiency in daily work activities.
Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).
Performs other duties as assigned.
