About The Position

Clinical Staff Pharmacists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory.

Requirements

  • Graduate of an accredited college or university with Pharmacy degree.
  • Licensed Pharmacist with the Kentucky State Board of Pharmacy in good standing.
  • Must provide documentation of state licensure and CE as required by the state of KY.
  • Maintain BLS & ACLS certifications

Nice To Haves

  • Pharm.D. or relevant clinical hospital pharmacy experience is desirable.
  • Advanced clinical training or PGY1 residency preferred

Responsibilities

  • Has a working knowledge of pharmacy operations including pharmacy operating system, use of patient records, purchasing, inventory and formulary management, automation, packaging, USP 797, USP 795, USP 800, and pharmaceutical waste.
  • Initiates, documents, and participates in department activities that promote positive patient outcomes.
  • Serves as a drug information expert and resource regarding medication management.
  • Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.
  • Participates in antimicrobial stewardship and opioid stewardship.
  • Provides medication counseling to patients.
  • Works with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patients for discharge.
  • Serves as an active member of the inter-professional team on patient care units.
  • Participates in unit-based huddles and activities if available.
  • Knowledgeable in unit’s metrics, goals, and works with staff on unit to help achieve goals
