About The Position

The Staff Pharmacist is responsible for providing quality pharmaceutical care/services to patients and health care professionals at any ARH or affiliated unit as assigned. This role involves properly assessing patients’ pharmaceutical needs, providing pharmaceutical care, and accurately compounding and dispensing prescribed medications and solutions. Key duties also include proper inventory and record keeping of medications, and counseling patients, their families, and appropriate health care professionals regarding drug therapy. This is an Inpatient pharmacist role.

Requirements

  • Bachelor of Science degree in Pharmacy, Pharm.D., or other advanced degree.
  • Licensure eligibility in state of employment.

Responsibilities

  • Supervises and performs functions involved in the drug distribution process, including accurate and complete order entry, interpreting and evaluating medication and intravenous orders for allergies, appropriateness, duplication and interactions, screening for, detecting, and documenting ADRs, supervising and checking the work of technicians, accurately filling medication orders/prescriptions, compounding non-sterile pharmaceutical products, counseling patients concerning medications (inpatient or outpatient), and pricing and billing prescriptions or medication orders.
  • Supervises and performs functions involved with the IV Admixture program, including preparing and labeling IV admixtures in accordance with accepted standards, screening IV preparation for compatibility, using appropriate aseptic technique in drug preparation, using safe and appropriate technique in chemotherapy preparation, supervising and checking the work of technicians, accurately filling IV orders, and maintaining IV Room area.
  • Makes appropriate interventions to improve patient care through effective communication, including accurately and efficiently performing physician consults (kinetics, dose calculations, wound care, poly-pharmacy, etc.), initiating interactions with physicians concerning drug therapy (recommendations, clarifications, non-formulary orders, age-specific considerations), documenting clinical interventions, assisting in establishing protocols and policies and procedures for appropriate drug use, formulating care plans and reassessing therapy as patient’s condition requires, responding to codes/emergency situations as required, providing current and accurate drug information to patients and other healthcare professionals, and providing both formal and informal educational services to patients, health care professionals, students, and community.
  • Demonstrates the necessary skills and proficiency needed to operate the Pharmacy computers and electronic and mechanical equipment, including printing computer lists and labels for respective Pharmacy area, showing proficient skill to operate and maintain Pharmacy computers, fax machine, copy machine, cash register, etc., maintaining laminar flow hoods, and maintaining, updating, and retrieving departmental files such as correspondence, licenses, prescriptions, profiles, cash receipts, records of issuance, etc.
  • Maintains appropriate record keeping and levels of medication and supply inventory, including demonstrating proper distribution and documentation of controlled substances (ordering, receiving, perpetual inventory, etc.), ordering or overseeing ordering of medications and supplies, and maintaining proper storage and distribution of floor stock and emergency drugs according to P&P.
  • Possesses the ability to assess and/or care for age groups of patients within their scope of practice - See age-specific competency guidelines for position and age group.
  • Serves on hospital committees as assigned.
