The Department of Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to, verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets. The Clinical Staff Pharmacist serves as a drug information expert and resource regarding medication management. They provide comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services. The role participates in antimicrobial stewardship and opioid stewardship and provides medication counseling to patients. They work with inter-professional patient care teams to help patients with transitions to care, including preparing patients for discharge. The pharmacist serves as an active member of the inter-professional team on patient care units, is knowledgeable in unit’s metrics and goals, and works with staff to help achieve them. They interpret and review medication orders for correctness and safety, ensure drugs are available to patients, oversee dispensing and reconciliation of medications, and perform medication reconciliation. The role provides technician oversight and assumes supervisory duties in routine daily activities. They supervise/perform production of compounded products and demonstrate supervisory skills to plan, organize, and direct activities in the pharmacy cost-effectively. The pharmacist assists in the orientation and training of pharmacy staff and students, and acts as a preceptor. They maintain knowledge of regulations and standards regarding pharmacy practice and medication management, effectively utilize pharmaceutical references, and promote formulary therapies by making recommendations to medical staff.
