About The Position

Provide advanced clinical pharmacy care including counseling and ongoing pharmacotherapy management for patients with complex specialty conditions. Perform comprehensive medication therapy management (MTM) to identify drug interactions and optimize therapeutic outcomes. Develop and monitor individualized care plans, tracking laboratory values, clinical progression, and adverse drug events. Provide evidence-based clinical recommendations to healthcare providers regarding drug selection, dosing adjustments, and patient safety strategies. Document in the electronic health record (EHR) and specialty pharmacy software to meet accreditation standards. Collaborate across interprofessional teams to streamline prior authorizations, medication delivery logistics, and care transitions.

Requirements

• Pharmacist obtained prior to hire date or job transfer date.
• Bachelor's or advanced degree in Pharmacy required.
• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
• Active, unrestricted State Pharmacist License or licensure by date of hire
• ASHP-Accredited PGY-1 Pharmacy residency or three (3) years of relevant clinical pharmacy experience required.
• Active Indiana license required by start date; remote role supporting Infectious Disease (HIV) clinics in the Indianapolis area; Role may require additional licensure in the future.

Nice To Haves

• Master's degree or Doctorate of Pharmacy preferred.
• ASHP-Accredited PGY-2 Pharmacy residency with a focus in ambulatory care or specialty pharmacy preferred.
