About The Position

NHC Healthcare Franklin is seeking a full-time Clinical Social Worker. The ideal candidate will possess a bachelor's or master's degree in social work, sociology, psychology, or a related humanities field. Experience in case management within a SNF/ICF or acute care setting is preferred, along with knowledge of MDS, clinical assessments, documentation, care planning, and discharge planning. Familiarity with support agencies such as hospice, home health, and mental health services, as well as 1:1 case management with patients and their support systems, is also desired.

Requirements

Bachelor’s or Master’s degree in social work, sociology, psychology, or related humanities
Experience with a case management background in a SNF / ICF or acute care setting
Knowledge of MDS, clinical assessments and documentation, care planning, discharge planning
Experience working with support agencies such as hospice, home health and mental health services
Experience with 1:1 case management with patients and their support systems

Responsibilities

Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
Working with individual patients and their representatives toward transition back to the community or adjustment to center life
Helping navigate a patient’s crisis of illness and / or disability
Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
Coordinating patient’s plan of care with interdisciplinary team members
Reporting to the Director of Social Work current case management status and needs
Maintaining confidentiality of professional information
