Requirements

  • MS Social Work from an accredited school of social work.
  • Must be licensed by the Maryland Board of Social Work Examiners at the LMSW level, but an LCSW-C is preferred
  • 2 years or more of licensed clinical social work experience in an inpatient Hospital healthcare setting.
  • ED experience preferred
  • Aidin Discharge software experience is strongly preferred

Responsibilities

  • Apply knowledge of case management practices, hospital policies, departmental procedures, and healthcare regulations to support high-quality patient care and outcomes.
  • Partner with the Case Management Coordinator (RN) to proactively identify patients at risk for extended hospital stays, complex clinical needs, high resource utilization, or repeat admissions, initiating interventions within 24 hours of admission.
  • Conduct comprehensive psychosocial assessments to evaluate patient and family needs, coping abilities, support systems, barriers to care, and discharge readiness.
  • Provide counseling, guidance, advocacy, and referrals to patients and families to address psychosocial and community resource needs.
  • Collaborate closely with physicians, nurses, interdisciplinary teams, and utilization management staff to develop and implement effective patient-centered care and discharge plans.
  • Identify and address barriers to safe and timely discharge, coordinating referrals to financial counseling, risk management, and other support services as needed.
  • Serve as a trusted resource for patients, families, caregivers, and community partners by providing education, information, and access to available services.
  • Coordinate comprehensive discharge planning, ensuring seamless transitions of care and appropriate post-hospitalization services.
  • Educate patients and families regarding community resources, including home health services, skilled nursing facilities, rehabilitation centers, and other post-acute care options.
  • Maintain current knowledge of community-based resources, healthcare regulations, and payer requirements that may impact patient care and discharge planning.
  • Monitor and facilitate implementation of discharge plans to ensure continuity of care and successful patient outcomes.
  • Complete and maintain accurate discharge planning, social services, and case management documentation in compliance with organizational and regulatory standards.
  • Initiate and maintain acute-care abstracts and MedQM data for assigned patients, ensuring timely and accurate reporting.
  • Promote efficient resource utilization while supporting patients in achieving the best possible clinical and psychosocial outcomes.
