About The Position

Requirements

• Master's degree in social work from a college accredited by the Council of Social Work Education
• Arkansas social worker license that is valid and in good standing with the licensing board
• 6 months of professional social work experience in a skilled medical environment such as home health, hospital, rehab facility, clinic or related
• COVID vaccine as a condition of employment (absent reasonable accommodations)

Responsibilities

• Provide rehab and supportive casework geared towards restoring patients to their optimal level of social, emotional & health wellness
• Help patients utilize appropriate family & community resources
• Refer patients to appropriate health & welfare agencies
• Consult & coordinate services with physicians and other members of the patient's care team
• Prepare clinical progress notes & incorporate into clinical record
• Participate in discharge planning & in-service programs
