About The Position

Alton Memorial Hospital is looking to hire a Clinical Social Worker, LCSW for the Emergency Department. This full-time position operates from 8am-4:30pm. The role provides clinical social work services to address the psychosocial needs of patients and families. This includes individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. The Clinical Social Worker collaborates with the health care team and ensures timely, cost-effective discharge and transition planning. The Case Management, Utilization Review, and Social Services department provides admission and continued stay reviews on all patients for the purpose of compliance and the proper utilization of resources, and provides social service resources to patients and their families.

Requirements

• Master's Degree of Social Work (MSW) – required
• LCSW Required
• Lic Clinical Social Worker

• 2-5 years experience

Responsibilities

• Conducts screening and assessment activities to meet the needs of the discharged patient.
• Applies assessment findings to prepare a patient-centered plan of care.
• Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
• Provides admission and continued stay reviews on all patients for the purpose of compliance and the proper utilization of resources.
• Provides social service resources to patient's and their families.
