About The Position

The Clinical Social Worker role involves providing comprehensive assessments, treatment planning, and counseling services to patients. This position is responsible for evaluating patient needs, coordinating discharge plans, and offering individual, family, and group therapy. The role also includes patient and family education, participating in interdisciplinary treatment planning, and providing on-call admission services. The Clinical Social Worker must maintain accurate clinical records in accordance with hospital policies and governing body regulations.

Requirements

  • Knowledge of population specific factors in assessment and treatment.
  • Knowledge of theories of human behavior and development.
  • Skills and ability in providing individual, family and group counseling.
  • Knowledge of community resources.
  • Advanced interviewing skills.
  • Knowledge of Social Work Code of Ethics.
  • Ability to make independent work decisions.
  • Ability to independently maintain clinical records on assigned cases in accordance with hospital policies and governing body regulations.

Responsibilities

  • Completing comprehensive assessments, social work reassessments, treatment plans, and other required medical record documentation in accordance with departmental policies, accrediting/certifying body regulations, and state/federal laws.
  • Assessing patient's discharge needs and coordinating discharge plans with the CSB, ensuring effective and efficient discharge planning according to policy and DBHDS discharge protocols.
  • Documenting the content of individual, family, and group sessions and providing education to patients and families according to departmental policy.
  • Participating with the interdisciplinary team in formulating individual treatment plans and functioning as the plan author for assigned patients.
  • Providing admission on-call services on a rotating basis.
  • Communicating with nursing supervisor, CSB, other facilities, Central Office, Jail, and others to assist with admission needs.
  • Consulting with patient flow manager and/or social work director regarding difficult admissions.
