About The Position

Starting as a land gift to the community from the Smith family, Alton Memorial today is a full-service acute care hospital. Alton Memorial offers patients a variety of inpatient and outpatient services, including surgery services, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24-hour emergency care, ambulance services and more. This role provides clinical social work services to address the psychosocial needs of patients and families. It involves individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. The role collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Requirements

• Master's in Social Work (MSW)
• Licensed Clinical Social Worker (LCSW) - IL
• Master's Degree - Social Work
• No Experience
• No Experience (Supervisor Experience)

Responsibilities

• Provides admission and continued stay reviews on all patients for the purpose of compliance and the proper utilization of resources.
• Provides social service resources to patient's and their families.
• Conducts screening and assessment activities to meet the needs of the discharged patient.
• Applies assessment findings to prepare a patient-centered plan of care.
• Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
