About The Position

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker to join their team. This full-time position works Monday through Friday, 8:30 a.m. – 5 p.m, and 1 weekend every six weeks. In this important role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care—directly enhancing the quality, dignity, and comfort of their hospice experience. The social worker visits hospice patients in the home, ALF, or SNFs in Belle Glade, Loxahatchee, West Palm Beach, Wellington. Trustbridge Hospice has cared for more than 200,000 South Florida families since 1978. As a community-based nonprofit, they provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

Requirements

Degree in Social Work or a related degree such as Psychology. Masters preferred.
One year of experience as a Social Worker. In a hospice setting is preferred.
Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.
Strong written and oral communication skills.

Responsibilities

Provide psychosocial assessment and intervention for patients and families within his or her scope of practice as a member of the interdisciplinary team
Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.
